Community Pharmacist Consultation Service

The NHS Community Pharmacist Consultation Service (CPCS) was introduced to facilitate patients having a same day appointment with their community pharmacist for minor illness or an urgent supply of a regular medicine, improving access to services and providing more convenient treatment closer to patients’ homes.

The service is helping to alleviate pressure on GP appointments and emergency departments, in addition to harnessing the skills and medicines knowledge of pharmacists. Should the patient need to be escalated or referred to an alternative service, the pharmacist can arrange this.

DAFNE – Dose Adjustment For Normal Eating

The DAFNE course is for adults (over 17 years old) with type 1 diabetes and is a comprehensive and often life-changing experience. It is far more than a carbohydrate counting programme, although learning and refining the skills necessary to count the carbohydrate in each meal and to inject the right dose of insulin are an important part of the course.

DAFNE aims to help adults with type 1 diabetes lead as normal a life as possible, whilst also maintaining blood glucose levels within healthy targets, to reduce the risk of long-term diabetes complications.

There are different ways in which you can complete a DAFNE course, either on a face-to-face DAFNE course or a Remote DAFNE course – a blend of online learning with virtual group sessions.

Face to face course: On a face-to-face course, you will attend a 5-day training course (either Monday to Friday, 9am-5pm, or one day per week for 5 weeks), with a follow-up session around eight weeks after the course. There are sessions on managing insulin around exercise, around illness, and around social activities including drinking alcohol. This is why the course might be longer than other ‘self-management’ training you might have heard of.

The face-to-face course is delivered to groups of six to eight participants under the supervision of DAFNE-trained educators (who are specially trained NHS diabetes specialist nurses and dietitians).

Remote DAFNE course: These were created in response to changes in diabetes services as a result of the Covid-19 requirement for shielding and social distancing.  It is also an option for people who feel that a face-to-face course is not for them.   Remote DAFNE courses enable you to learn at home; fitting around your work and home commitments.  The Remote DAFNE course will take 5 weeks to complete. It includes online learning from home each week and weekly group video support calls with up to 4 other participants and a trained Remote DAFNE educator.

The DAFNE course, in whichever format, is about learning from experience, trying new things under supervision and benefiting from the support of other people on your course.  During the course you practise the skills of carbohydrate counting and insulin adjustment around lifestyle activities.

NHS Health Checks

We are offering Face to Face health clinics for adults 40-74, who do not have any of the following pre-existing conditions:

  • heart disease
  • chronic kidney disease
  • diabetes
  • high blood pressure (hypertension)
  • atrial fibrillation
  • transient ischaemic attack
  • inherited high cholesterol (familial hypercholesterolemia)
  • heart failure
  • peripheral arterial disease
  • stroke
  • currently being prescribed statins to lower cholesterol
  • previous checks have found that you have a 20% or higher risk of getting cardiovascular disease over the next 10 years

  • NHS Health Check looks at your general health recording your height, weight, age, sex, ethnicity, blood pressure, cholesterol and HbA1c.
  • 20-30 minutes appointments available during the day, weekend and evenings.

  • Help to get specialist support if needed.

Eastern European Health Trainer Service

We are offering Face to Face health clinics for adults to provide advice and support on lifestyle factors such as healthy eating, weight management, exercise and alcohol management to improve your health and wellbeing. The clinics are held by Healthy You Advisors.

Points for the programme:

  • Motivational behavioural change support for up to a year, in your national language we will engage with translator services if the Health Trainers do not speak your language.
  • 6 sessions of 1:1 healthy lifestyle support, 60-minute Initial assessment, 30 minutes thereafter.
  • Appointments are booked to suit you, you can choose weekly, fortnightly or monthly appointments.
  • We will support you with:
    • Healthy eating
    • Losing weight
    • Increasing physical activity
    • Stopping smoking
    • Reducing stress
    • Cutting down on alcohol
    • Sexual health concerns

Eligibility:

Residents within Fenland & Peterborough aged 16 + from an Eastern European background who want to make a Healthy Lifestyle change.

Think you might be drinking too much?

There are many reasons why people drink; relaxation, stress, habit, fun, pain management, peer pressure, loneliness, to forget the list goes on. If you feel your drinking is becoming a problem, or you’d simply like to reduce your alcohol intake, or go alcohol free for a while, we are here to help.

The Government advises that adults – both men and women – should drink no more than 14 units a week. But it can be hard to know exactly how much a unit is, especially if we are drinking at home, as we all pour bigger measures.

Why cut down on your alcohol intake

Cutting down on how much alcohol you drink can have some positive effects on the way you look and feel – often within just a few days.  You will sleep better, have brighter skin and more energy and more importantly you will reduce your risk of developing serious illnesses like cancer, liver and heart disease.

How we can support you

We know it can be hard to keep track of how much you’re drinking.

Cutting down on how much you drink isn’t always easy, but you won’t be alone. We will help you understand more about your relationship with alcohol, what safe levels of drinking are and help remove the confusion around the number of units.

We can refer you  to support you on your journey to living a healthier happier life, alongside alcohol reduction we can look at other areas of behaviour change if you’d like to, including;

  • Healthy eating
  • Losing weight
  • Increasing physical activity
  • Reducing stress

One of Alcohol Health Trainers will work with you, meeting regularly to help you achieve your goals.

We understand many people are anxious about the impact of Coronavirus (COVID-19) on their ability to access treatment and care.

If you have any signs or symptoms of cancer, you may be worried about whether you will be able to see a GP or a specialist and get any tests you need. You may also be worried about whether it is safe to do so. Please do still contact your GP straight away. If you do have cancer, this will help you to get a diagnosis as soon as possible.

Cancer teams throughout the UK are doing their best to maintain services. They want to ensure that cancer diagnosis, treatment and care continues as much as possible.

Health Coaching

Health and wellbeing coaches help you make positive and lasting changes to your health. The coach guides the individual through the process of creating a vision for their health and well-being, developing a healthy mindset and healthy habits, and encouraging you every step of the way until you accomplish their goals.

Seven of our Healthcare Assistants who speak a number of languages have been trained as health Coaches to help provide support to our patient population we also have health coaches and health trainers from the council who work with this team from within our practice

They focus on some specific different areas: Exercise, Sleep, Wellbeing, Diet, Smoking and Alcohol consumption.

This service is available for any patient registered with Thistlemoor Medical Centre. We will be helping anyone who wants help with lifestyle factors, patient referred by a health professional, with a long-term condition such as a respiratory or heart condition.

You will be supported to make changes to your life and prioritise improvements that are important to you. This will be monitored through questionnaires at the beginning and at the end of treatment. This is supported by a GP lead – Dr Emma Hamilton.

We have been reaching out to local contacts in Physiotherapy (Ultimate Performance Lifestlye – (https://www.upl.life) who are putting together some video’s to help people stay more active. We have also worked with a local Personal Trainer (Giuseppe Polvere) who has put together information on Nutrition and Staying Active with exercise. We are interested in working with local dieticians and if you are available and would like to be part of this work – please contact us.

NHS Diabetes Prevention Programme

NHS Diabetes Prevention Programme

The Healthier You: NHS Diabetes Prevention Programme (NHS DPP) identifies those at diabetes high risk and refers them onto a behaviour change programme.

Eligibility:

18+ with pre-diabetes (HbA1c 42-47mmol/mol or fasting plasma gluc 5.5-6.9mmol/l tested within the last 12 months

• Focus on diabetes prevention, but includes weight management.  9 months with 13 x 1.5 hour sessions.   No practical physical activities in sessions.
